Appointments
Your scheduled appointment time has been reserved specifically for you. We request 24-hours notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment. After missing your second appointment without notifying us 24 hours in advance, you are subject to being charged an additional fee.

Insurance
Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid at the time of service. As a service to our patients, we will bill insurance companies for services and allow them 45 days to render payment. After 60 days, you are responsible for the entire balance, paid-in-full. If you have any questions, our courteous staff is always available to answer them.

Patient Referral Incentive
Patient will receive 150 Orthoperks Reward points for every patient that they refer that starts treatment. You will receive a $100 Visa gift card or $100 off your treatment price after you refer three patients who start orthodontic treatment with us.

New Late Policy
As a professional and courteous office, we believe it is important to respect and value our patient’s time. In return, we ask that each patient have the same regards for other patients. In order to keep wait times in our office to a minimum, we have implemented a late policy. We appreciate your understanding and consideration as we work together to make Jolley Smiles an even better practice. Thank you for being the best part of our practice!!

Please take a few moments to read the following policy:

-If you are more than 5 minutes late to your scheduled 15 minute appointment, we will have to reschedule your appointment.
-If you are 10 minutes late to your 30 or 45 minute scheduled appointment we will have to reschedule your appointment.
-If you are 15 minutes late to your 60 minute scheduled appointment we will have to reschedule your appointment.
